(CRSI) doing business as Integra Rehabilitation is a Georgia nonprofit corporation organized for charitable purposes within the meaning of Section 501(c)(3) of the Internal Revenue Code of 1986, as amended (the “Code”). CRSI provides physical, occupational and speech rehabilitation and related health care services to members and non-members of an integrated healthcare delivery system.
